All athletes grades 7-12 must also participate in the Temple Independent School District, Mandatory Random Drug Testing Program. Below you may also also download the following drug testing forms in PDF format.

An athlete may request a copy of their physical form if they are transferring to another school or may be involved in a camp from the Athletic Training Staff. Records are considered private and will only released after a parental conscent form authorizing the release of such information is signed and turned into the athletic department. If you are requesting a release of your medical history and physical please fill out and sign the medical release form printable in a PDF format below. Forms may be turned into the Athletic Department attention Windee Skrabanek or Mike Lefner.
