Immunizations
Required immunizations include:
Oral Polio
Students
4 years and older are required to show proof of four doses of polio vaccine
one of which must have been received on or after the fourth birthday.
Or, if the third dose was administered on or after the fourth birthday,
only three doses are required. If any combination of four doses of OPV
and IPV was received before four years of age, no additional dose is required.
Children enrolled in school (grades K-12) prior to August 1, 2004, and
who received a booster dose of polio vaccine in the calendar month of
or prior to their fourth birthday, shall be considered in compliance with
polio series.
Tetanus/Diptheria
Students 14 months and younger are required
to have three doses of DTP/DTaP vaccine. Students 15 months though 4 years
are required to have four doses. Students entering kindergarten
are required to have five doses of diphtheria-tetanus-pertussis containing
vaccine, one of which must have been received on or after the fourth birthday.
Or, if the fourth dose was administered on or after the fourth birthday,
only four doses are required. Students seven years and older are required
to have at least three doses tetanus-diphtheria containing vaccine, provided
at least one dose was administered on or after the fourth birthday. Any
combination of three doses of a tetanus-diphtheria contain vaccine will
meet the requirement. Children enrolled in school (grades K-12)
prior to August 1, 2004, and who received a booster dose of DTaP vaccine
in the calendar month of or prior to their fourth birthday, shall be considered
in compliance with DTaP series. One dose of tetanus-diphtheria containing
vaccine is required with in the last ten years. This also applies to pregnant
students.
Measles (Red)
All students four years and older are required to
how proof of two doses of measles containing vaccine. The first shall
be administered on or after the first birthday. The two doses of measles
vaccine must be administered at least 28 days apart. Serologic confirmation
of immunity to measles is acceptable in lieu of vaccine. Evidence of measles
illness must consist of a laboratory report that indicates either confirmation
of immunity of infection
Mumps
One dose of vaccine given on or after
the first birthday is required for all student under 17 years of age or
serologic confirmation of mumps immunity or serologic of infection.
Rubella
One
dose of vaccine given on or after the first birthday is required for all
students or serologic confirmation of rubella immunity or serologic evidence
of infection.
Pneumoccal Conjugate (PVC)
All students
under the age of 5 are required to have at least one dose of pneumoccal
vaccine on or after 12 months of age.
Haemophilus Influenza (HibCV): One dose of Hib vaccine given on or after 15 months of
ages required for students 15 months through for years of age, unless
a primary series and booster ( a total of four doses) have been completed
prior to or at 15 months of age.
Hepatitis A
All
students under age 5 are required to have two doses of hepatitis A vaccine
separated by at least 6 to 18 months. HEPATITIS B: All students
are required to show proof of three doses of hepatitis B vaccine. Serologic
confirmation of immunity to hepatitis B is acceptable in lieu of vaccine.
Evidence must consist of a laboratory report that indicates either confirmation
of immunity of infection.
Varicella
All students are required
to show proof of one dose of varicella vaccine given on or after the first
birthday. 2 doses are required if the child was 13 years old or older
at the first dose of varicella vaccine was received. A parent or physician
validated history of varicella disease (chickenpox) or varicella immunity
is acceptable in lieu of vaccine.
Tuberculosis (TB)
All
students entering the Temple ISD from a foreign country shall provide
evidence of having received a TB skin test. Students with a positive
TB skin test of 10mm or more and a negative chest x-ray will be allowed
in school. A doctor’s note will be required if there
are any other symptoms or questions of communicability.
A student may
be excused from taking required immunizations if he/she has an affidavit
signed by a doctor that the immunizations would be injurious to the student’s
health, or an affidavit signed by his parent or guardian that the immunizations
conflict with the tenets of a recognized religious denomination of which
the pupil is a member. A time limit of one year is set for
medical contraindication statements unless the physician specified a lifelong
condition.
To claim exclusion for medical reasons, students must
present a statement signed by the child’s physician who states the
vaccine is medically contraindicated or poses a significant risk to the
health and well being of the child or any member of the child’s
household. Unless it is written in the statement that a lifelong
condition exists, the exemption statement is valid for only one year from
the date signed by the physician. To claim an exclusion for reasons
of conscience, including a religious belief, a signed affidavit must be
presented by the child’s parent or guardian stating that the child’s
parent or guardian declines vaccinations for reasons of conscience, including
because of the person’s beliefs. The affidavit will be valid
for a two-year period.
The child, who has not received the required
immunizations for reasons of conscience, including religious beliefs,
may be excluded from school in times of emergency or epidemic declared
by the commissioner of public health. An affidavit form may be obtained
by submitting a written request to the Texas Department of Health in Austin,
Texas. To obtain an exclusion for armed forces, persons who can
prove that they are serving on active duty with the armed forces of the
United States are exempted from the requirements.
