There will be an athletic trainer available to accept forms / paperwork / doctor notes:
Tuesday- Thursday (July 27 -29)
7:30am-9am
Field House Athletic Training Room

*Just a reminder you must be cleared to participate in Summer S&C workouts and all activities after Aug. 2nd.

*Yellow Medical History form filled out and signed by parent/guardian, will need to turn into athletic trainer to be reviewed. (you can pick up forms at the athletic training rooms).

*Online forms must be completed each year by parent/guardian. Use the following link to access our online forms:
