Opportunity to turn in paperwork (Medical History/Physical).

Athletic Trainers will be at Temple High School to accept paperwork on Thursday, Aug. 18th
7:30am-12pm
Gym Athletic Training room, (inside the back area of the main gym).

 ***Medical forms can only be turned into the athletic trainer. (Medical History & online forms must be completed each year).
